Wry Neck
A wry neck is a sudden, painful condition where the neck becomes stiff and stuck in a twisted and/or tilted position. It often appears without warning – commonly after sleeping in an awkward position, a sudden movement, or sometimes with no obvious cause at all. People typically wake up unable to turn the head fully, with sharp pain and muscle spasm on one side of the neck.
Cervical Spondylosis
Cervical spondylosis refers to age-related changes in the structures of the neck – including the intervertebral discs, joints and bones of the cervical spine. This is a common condition that affects most people to varying degrees, particularly from age 40 onwards. The reduced ability of the intervertebral discs to absorb shock can lead to increased stress on the cervical facet joints and promote formation of bone spurs (osteophytes) – these changes contribute to stiffness, neck pain and reduction in movement. In some cases, cervical spondylosis can also cause nerve root compression or narrowing of the spinal canal (spinal stenosis), resulting in symptoms including arm pain, numbness, tingling, or even coordination issues in the hands. Facet Joint Arthropathy
Cervical facet joint pain is one of the most common causes of mechanical neck pain, particularly in people over the age of 40. It’s often felt as a sharp and/or aching pain on one side of the neck, occasionally radiating into the shoulder or upper back. Movements that require rotation and extension are often stiff and painful.
Cervicogenic Headaches
Cervicogenic headaches originate from dysfunction in the structures of the upper cervical spine – particularly the joints and muscles around the top three vertebrae (occiput-C3). These headaches often present as a steady, dull ache that can start at the base of the skull and radiate to the temples, forehead or behind the eyes. They are commonly one-sided but can shift sides over time. Neck stiffness, tenderness at the base of the skull and worsening symptoms with neck movement or sustained postures are the main symptoms reported.
Cervical Radiculopathy
A disc bulge occurs when the soft material inside the intervertebral disc imprints against the outer layer of the disc wall, irritating nearby nerve roots. This can cause local neck pain as well as radiating symptoms into the shoulder, arm, hand or thoracic region. The evidence suggests that cervical disc pathology is a common cause of nerve root compression especially in people aged 30-50, and is often associated with repetitive loading, prolonged postures or degenerative changes in the spine.
Whiplash Associated Disorders
Whiplash injuries commonly result from sudden acceleration-deceleration forces, such as those experienced in a motor vehicle accident, contact sports or falls. These forces place rapid stress on the neck that may lead to injury in the muscles, ligaments, joints, intervertebral discs and nerves. Whiplash refers to a wide range of symptoms that can follow such an injury – whilst neck pain and stiffness are most common, other symptoms may include headaches, shoulder and upper back pain, dizziness, visual disturbances, difficulty concentrating, photophobia, phonophobia and fatigue. These symptoms can vary in intensity and duration – for some, recovery is relatively quick, while others may experience persistent issues that impact daily life. Evidence shows that early intervention with a structured rehabilitation plan are key to a successful recovery.
